The only one I've ever seen is Alpine Gig > Montage > Gansta PArty, and I think Vasek, Clavens and Penske have hit it.

In Worlds 2005, I asked Vasek if he had hit a Jackpot. He said that after Euros, before coming to Worlds, he had hit

Alpine Whirlygig > Montage > Gangsta Party > Alpine Food Processor

And this was already in 2005!

Felix has also hit Jackpot, which I believe was Gyro Whirlygig > Montage > Gangsta Party.
